1. I understand that participating in an exercise class involves risk of injury; I agree to be solely responsible for any injuries sustained by me as a result of my participation in this class or any future classes I take at Tiny Little Movements. I’m fully aware of the risk involved.
2. I understand that it is my responsibility to consult with my physician prior to and regarding my participation in any classes offered at Tiny Little Movements. I represent and warrant that I have no medical conditions that would prevent me from participation in any classes offered at Tiny Little Movements.
3. BABY DANCE CLUB: I understand that participating in a movement class involves risk of injury; I agree to be solely responsible for any injuries sustained by me or the child under my care as a result of my participation in this class or any future classes I take at Tiny Little Movements. I’m fully aware of the risk involved.
4. I understand that classes I participate in may be filmed and or/photographed with the intent of distributing these images or video on social media or the Tiny Little Movements website. I agree to having my (and/or the child under my care at the time of attendance) image displayed on Tiny Little Movements social media accounts and/or the Tiny Little Movements website.
I assume full responsibility for any injuries or damages, known or unknown, which I might incur as a result of participating in any classes offered at Tiny Little Movements. I knowingly, voluntarily, and expressly, waive any claim I may have against Tiny Little Movements for injuries or damages that I (or the child under my care) may sustain as a result of participating in any of the classes offered.
